How To Make A Claim
To make a claim simply follow the steps below. All valid claims will be paid once the necessary documents are
received, without delay.
- You should call us on 1300 73 7697 or email service@reallifecover.com.au
and let us know that you will be lodging a claim. We will provide the necessary documents and assist with the
process.
- Complete the following forms and provide the following documents:
- Claim Booklet – Income Protection Cover
- A certificate of proof of identity (e.g. Birth Certificate, Passport, Driver’s License)
- A certified copy of proof of your identity to the deceased (e.g. Passport, Driver’s License, Bank Card,
Medicare Card)
- If you would like the benefit amount paid directly into a bank account you can also include a completed Direct
Credit Authority
- The original Policy Document and Policy Schedule
- Ongoing benefits will be subject to completion of the Claim Booklet.
