How To Make A Claim
We aim to pay benefits for valid claims, subject to the waiting period selected by you as soon as we receive your completed Claim Form together with the required supporting documents. Accidental Death claims will be paid as soon as proof of Accidental Death is received.
Family Life, Funeral Expenses, Serious Illness, Accidental Death and Serious Injury and Silver Life Cover
- Your beneficiary, family member or yourself should call us on
1300 73 7697 or email service@reallifecover.com.au and let us know that you will be lodging a claim. We will provide the necessary documents and assist with the process.
- Complete the following forms and provide the following documents:

Claim Booklet
-
A certified copy of evidence of death
-
A certified copy of evidence of deceased’s age
-
A certified copy of proof of your identity and your relationship to the deceased
-
The original Policy Document and Policy Schedule.

If you would like the benefit amount paid directly into a bank account you can also include a completed Direct Credit Authority

Income Protection Cover
- You should call us on 1300 73 7697 or email service@reallifecover.com.au and let us know that you will be lodging a claim. We will provide the necessary documents and assist with the process.

A certificate of proof of identity (e.g. Birth Certificate, Passport, Driver’s License)

A certified copy of proof of your identity to the deceased (e.g. Passport, Driver’s License, Bank Card, Medicare Card)

If you would like the benefit amount paid directly into a bank account you can also include a completed Direct Credit Authority
- The original Policy Document and Policy Schedule

Ongoing benefits will be subject to completion of the Claim Booklet.
