To make a claim simply follow the steps below. All valid claims will be paid once the necessary documents are
received, without delay.

Download a Pet Insurance Claim Form
- Complete and sign the form
- Take the claim form to your vet, and have your vet complete the applicable sections
- Mail your completed claim form and original receipts to:

All claims should be submitted
and received within ninety (90) days of the Vet Treatment being provided. Claims not received within this period will incur a late submission fee.
